Referrals, Admissions Process and Criteria
Survivors of traumatic brain injury (TBI), neurologic injury, and individuals with neuropsychiatric disorders are referred to Lakeside Neurologic from Florida and across the United States. Each patient is qualified for admission on an individual clinical, medical, and financial basis to ensure the most appropriate level of care and rehabilitation support.
Family members and caregivers are encouraged to tour the program prior to admission to make informed placement decisions. Our Admissions Department is always available to answer questions, explain the process, and help you find the best fit for your loved one’s recovery journey.
Referral Process
Who Can Refer a Client for Rehabilitation?
Referrals can be made by anyone involved in a patient’s care or support network, including:

Physicians, therapists, or other healthcare professionals

Hospital discharge planners and social workers

Insurance case managers

School representatives or community agencies
Lakeside Neurologic accepts local referrals from Florida and national referrals from providers and families across the country.
What Information Is Needed to Start a Referral?
To begin the referral process, please have the following information ready:
- Family contact or guardian information
- Client details, including age, injury type, and current location
Providing complete information allows our admissions team to review cases quickly and determine eligibility for neurologic or brain injury rehabilitation services.

How Long Does the Process Take?
